We recently changed the process for requesting new computer equipment from Information Technology Services (ITS). We have moved the request form from InfoPath to our JIRA Service Desk Portal.

Step by Step guide

  1. In any web browser navigate to the IT Services Portal and log in using the same credentials you would use to login to any CW Windows computer.
  2. Select "New Equipment request"
  3. Fill out the form.  Be sure to follow all instructions next to each field so that your request can be processed promptly.
    1. "Raise this request of behalf of" will automatically populate with your name.  Be sure to change it if you are requesting for another user.  All approvals are based off of the user in that field.
  4. Be aware that the "Hardware Requested" field will automatically open additional fields that will need to be filled out as well.  Each additional field opened will clarify the specific equipment needed.
  5. Click the "Create" button.
  6. Your request has been submitted.  It will automatically notify the appropriate managers and financial liaison for approval and then be ordered by ITS.  


You can monitor the status of your equipment request from approval to installation by visiting IT Services Portal and clicking on "Requests" at the top right of the screen.
